IN-DUES AD & D PLAN (no cost to members)
All dues-paying members of the Alabama State Employees Association are provided with $5,000 in Accidental Death and Dismemberment coverage, which includes a matching common carrier rider. This is at no cost to the member. ASEA pays the premium for this insurance as an added benefit of membership. No application is necessary. You are automatically enrolled.

VOLUNTARY AD & D COVERAGE
Accidents can happen anytime, anywhere, and they can be devastating. Few families are prepared to pay the additional cost necessary to financially support their loved ones after an accident. With that in mind the ASEA has made available this affordable Accidental Death and Dismemberment insurance for its members at low group rates. The plan pays up to $250,000 for death or accident. It includes an escalator benefit to help offset inflation. A Common Disaster benefit for you and your covered spouse if both suffer accidental death in a common accident. A Seat Belt benefit that pays up to $25,000 if you or your covered dependents suffer accidental death while wearing a properly fastened, factory-installed seat belt. VISION
The cost associated with quality vision care including frames, lenses, and contacts has increased substantially over the past several years. The plan offers you freedom to choose a different participating provider for each member of your family and low out-of-pocket expenses when selecting an in-network provider. The plan includes benefits for lenses, frames, contact lenses, and comprehensive vision exams among the other benefits provided. This vision plan is available by special offer only. CANCER PROTECTOR
The cost associated with the treatment of cancer, over and above what your health insurance pays, can be substantial. With that in mind the ASEA has worked with Countryman & Smitherman Inc. and Fidelity Security Life INsurance Company to bring you a very low cost plan of insurance that most everyone can afford. This benefit is designed to supplement your other health benefits. There are three plans to choose from (1) low option plan with a $500,000 maximum lifetime benefit per insured, (2) high benefit plan with $1,000,000 maximum lifetime benefit per insured, (3) a First Occurrence cancer benefit that pays $10,000 for the first diagnosis of internal cancer. The First Occurrence benefit is available only through special offers usually one time per year. Benefits are paid directly to you, in addition to any other insurance you may have. There are also benefits for cancer screening with no cancer diagnosis necessary. You owe it to yourself to look at these rates. We invite you to compare them to any cancer plan offered to state employees. GUARANTEED ISSUE TERM LIFE
The newest addition to ASEAs term product line is a $10,000 Guaranteed Issue Term Life plan available to members of the ASEA only. GROUP TERM LIFE
Thanks to your valuable membership in ASEA, you can take advantage of this Term Life Insurance Plan at affordable group rates. We invite you to compare these outstanding unique features and price to other term life plans available to you. All ASEA members and their spouse under age 65 and dependent children to age 23 can be covered. You choose the beneficiary and amount of coverage up to $260,000. Smoker and non-smoker rates are available. This plan includes extra benefits at no additional cost to you. Depending on the plan selected, benefits can include:

SENIOR WHOLE LIFE
The ASEA recognizes that there are situations when a persons health history or age greatly limits the options available to them for life insurance to protect their families financially. For those members or their spouses that might have problems getting insurance or for members that would rather have a permanent coverage that does not decrease or terminate due to age, Countryman & Smitherman Inc., offers a guaranteed issue plan of Whole Life Insurance. This plan is underwritten by Fidelity Security Life Insurance Company (FSL) and has been providing benefits for ASEA members for 23 years. The plan can be issued to ASEA members between the ages of 45 and 75, and offers up to $20,000 in valuable modified whole life coverage, paying two times the face amount if death is due to accident. For death due to natural causes, the plan is limited to 125% of annual premium during the first year, 250% of the annual premium during the second year, and the full-face amount during the third coverage year and beyond.
